Becoming a Patient

[text_block style=”style_1.png” align=”left”]When you schedule your initial office visit, you will be asked to download and fill out our new patient forms below, and bring them with you to your appointment.

If you are currently taking any medication and/or supplements please bring them with you to you initial visit so we know exactly what you are taking.

If you have any current lab test results (within the last 6 months). If you do not have any copies, we are happy to have you fill out a records release and get those at a later date.

Please arrive 10-15 minutes before your visit so we can get your paperwork ready.

Your first patient appointment is 60-90 minutes.[/text_block]
